Community Health Care Assistant (Preston County)

LOCATION: Preston County

HOURS:  Various Part-Time Positions Various hours, shifts and days
Flexible around school schedules for students looking to gain experience in the human services field


DIVISION:  I/DD

JOB DUTIES: In-home skill development training with I/DD individuals under the supervision of a Therapeutic Consultant

  • Accountable for personal needs (i.e. grooming, bathing, dressing, nutrition).
  • Documentation as required by Valley, Medicaid, OSHA, and other licensing agencies. Employee will ensure that documentation (such as Progress Notes, Data Sheets, SAL’s, Timesheets, Task Analysis Forms, Behavior Tracking Forms, DD Forms, etc.) are completed accurately and timely and will reflect services the client(s) has received as appropriate. 
  • Perform all-predetermined services as per doctor’s orders. Assure that the safety, health and well being of consumers are always maintained on or off site.
  • Implement treatment/behavior/ plans in compliance with Valley’s and State guidelines.
  • Employee will engage clients in achieving their daily goals and objectives through active treatment.
  • Employee will ensure that personal care, change in client’s health status and hygiene of all clients is maintained as client needs dictate.

Employee will complete all job duties as assigned within the specified timeframe or shift as scheduled. In doing such duties, all Valley Policies and Procedures as well as State and Federal regulations will be complied with in a promptly and timely manner. Examples of such standard duties include but are not limited to completing shower schedules, dressing and changing clients appropriately, preparing and following meals plans and schedules, notifying the supervisor of items needed by the clients, completing shift to shift reports, following cleaning schedules, 

 

QUALIFICATIONS: High school diploma or equivalent the ability and dependability to legally transport clients        

Prefer: Prior experience working with the I/DD population or in a healthcare setting
